Insurance & Payment Information
Please see here for the full announcement letter in regards to insurance participation. Dr. Menn is considered an "out-of-network provider" with most plans. Payment is expected at the time services are rendered. All labs and paps are sent to a lab that is covered by your insurance, so the only fee is Dr. Menn's professional fee for the office visit. Most patients see Dr. Menn 1-2 times per year, so the out of pocket costs are not high. Reasonable fees are based on a discounted rate from Healthcare Bluebook.
Dr. Menn's Fee Schedule
We will give you a properly coded receipt for you to submit yourself for reimbursement , or to an HSA. Please ask your insurance company about reimbursement policies for care rendered by out-of-network providers as this will vary by plan. Upon request, we can also submit the receipt electronically to most insurance companies for you.
For Medicare patients: Due to dwindling reimbursements by Medicare to providers , Dr. Menn has elected to opt out of Medicare. In order to see a physician who has opted out, Medicare requires patients to sign a "Medicare Opt-Out Contract", confirming that they agree to be seen on a private-pay basis by that physician. This contract does not prohibit the individual from seeing other physicians who are participating Medicare providers. In addition, neither the patient nor the doctor is allowed to bill Medicare for the visit. The patient must sign this contract at to the visit, and payment for services rendered will be expected at the time of the visit.
We accept cash, check, and all major credit cards. Payment is expected at the time of the visit.
